Confirmed regressions, not noise

A new scan is only compared against the last successfully completed scan - a transient crawl failure is never mistaken for a verified fix, and a real regression reopens the original finding instead of creating a duplicate.

Every score is tracked over time, by website, department, and document type, so you can see whether the program is actually improving.
